Our functional medicine model of care at Hopkins Health and Wellness offers a patient-centered approach to chronic disease management. It seeks to answer the question, “Why is a patient ill?” so they can receive personalized, effective care for their needs.

Our doctors will spend time listening to patients and gathering their medical history. Using this information to identify the root cause(s) of their illness, including triggers such as poor nutrition, stress, toxins, allergens, genetics and their microbiome (the bacteria living in and on the body).

Once the triggers are identified, we customize a healthy living plan for each patient. Their plan will address many aspects of their life, from physical needs, including nutrition, exercise and sleep, to mental and emotional stressors related to social, work and community life.
